Update- Dealer called today and said a wrist pin flew up and caused the hole in the engine. They said they have never seen it before.. waiting on Toyota rep to okay approval for new engine replacement.

My 2019 Toyota Tundra broke down on me Friday as I entered the highway. I had to quickly drift into the highway median- the steering would not move and then engine cut off. I got out and there was oil all over the ground of the highway. I had it towed to dealer- they called me and said there is a hole in the engine. There were no lights on the dash and the truck is serviced at the dealer every 5k miles and we bought maintenance and extended warranty for it. The service advisor called and told me yesterday the mechanic said one of two things happened: we drove it in a body of water and got water in the engine or the heads are warped- he said the oil is milky.
I have not driven that truck in a body of water period. They are waiting on Toyota rep to come in and see if they can cover under the 5yr 60k warranty or next step is extended warranty. The truck was about 400miles out of needing the next service. The truck currently has 48,409 miles. Anyone have any issues like this? They said I will need a new engine. I added the picture of the engine with the hole they sent me

Are they saying you are passed 5 years? That is a strange looking hole.Looks like a knockout almost. Blaming you driving through water which didn't happen you said is trying to get out of the repair honestly.
I have a 21 with 40K on it and no problems.

Looks like a rod broke and played peek-a-boo with the engine block, sometimes it happens, any mechanical man made thing has failures. Wouldn’t put any thought into what the dealer mechanic said, 1 - takes maybe 2 minutes to open the air box and inspect the box and filter for water intrusion, 2 - of course the oil is milky, the inside of the motor is on the outside. Hopefully the Toyota field rep will get it sorted out for you.

Milky oil is likely bad head gasket. Should have had some codes for misfire or overheating though. Very unusual failure for a 5.7.

I don't think any sensor can detect an open hole on the side of the engine block and the oil making a quick exit... You most likely overheated during that oil starvation period which caused coolant intrusion. I'm not a mechanic just an assumption, like @Vizsla stated it takes a few minutes to check the airbox to check off any water lines or intrusion. Mechanic's job is to assume the owner screwed it up just wait on what the Toyota Rep will say. If it was dealer maintained like you stated, you should be covered, unless someone there added coolant to the wrong place. Wish you the best.

The codes would precede the open block occurrence. Bad head gasket would allow oil and coolant to migrate to places it shouldn’t. Each other’s passages, or into the cylinder itself. Hydrolock could cause catastrophic failure. End result, not the initial problem.
