Accelerator and Brake Pedal pressed Simultaneously error code

    Hello Everyone.

    I have a 2019 Tundra with a SmartCap and AlphaRex headlights installed last December.

    Recently I've been getting the warning message "Accelerator and Brake Pedal Pressed Simultaneously"

    Upon quick acceleration from a stop, and sometimes at lower speeds, the truck will begin to accelerate and then just cut out, whereby I have to take my foot off the pedal and feather it quickly to begin moving again.

    It's definitely not from pressing both pedals at the same time.

    Very dangerous situation in traffic.

    I've read on forums that it could be something as simple as a brake light bulb or improper wiring from the cap dealer.

    The truck has been at the dealer for 4 days and they are going over the whole wiring system and of course cannot duplicate the issue. Luckily they were able to print out 2 pages of this error code from the truck.

    If anyone has any advice as to what may be causing this issue, I would be very appreciative.

    Still happening.
    I'm bringing it back to the dealer for the 3rd time on Monday.
    Tech adjusted the stop lamp switch on the brake pedal which worked for a week.
    Cap wiring is ok.
    Going to have them change the brake bulbs and stop light switch light assembly since that seems to be the most prevalent solution I've found on older forums.

    UPDATE 7/8/24
    So Toyota checked all the electrical and wiring. Three days there and they could never fix the problem.
    This continued until one day my brake lights would stay on and I couldn't turn them off no matter what.
    My neighbor who can pretty much fix anything looked underneath the dash at the brake pedal assembly.
    Apparently the cheap plastic brake plunger that sits in between the two heavy metal plates had become dislodged a bit and was askew causing the plunger to be stuck in the depressed position. He said that sometimes your foot can hit it and move it out of place.
    My neighbor took it out to show me how cheap it was and installed it back properly.
    I thought it just fixed the brake light always on problem, but since then, the truck has never again flashed the "Accelerator/Brake pressed simultaneously" warning light and gone into limp mode.
    I never heard of this being a fix no matter how much I researched the problem, but it did.
    Hopefully this may be of help to someone else and they don't have to go through what I did to fix it.

    Would you happen to have a picture of the clip? My 2021 tundra just did this to me in traffic. It was interesting to say the least.

    I don't have a photo, but all my neighbor did was take out the plastic plunger between the metal brake plate and spacer under the dash. Make sure the spacer was in place correctly and not broken. See video below.

    It's a cheap, plastic plunger that presses against the spacer.

    It's easily accessible and he put it back in correctly. It was not seated perfectly in it's slot. I would just take it out and put it back in making sure the spacer is good. If that is the problem.

    I read that others have had the problem due to:
    *faulty rear brake lights bulbs. Need to replace them
    *installed LED bulbs
    *3rd light bulbs or improper wiring from a cap.

    Toyota went over my whole electrical system and everything was ok. So, for me it was the plunger and spacer not being aligned correctly, thus sending a signal to the system that the "Accelerator and Brake pressed simultaneously" causing the warning light and truck to go into limp mode.
    I would let off the accelerator and feather it to get the truck going again.
    I hope you find the issue because it causes a dangerous situation.
