Rear brakes, 2012 tundra

Did my front and rear brakes about 1.5 years ago.

As I did a few hard stops recently I heard a loud grinding sound.
When I had installed the brakes 1.5 years ago I had issues with the clips making noise and it was a pain to mess with the clips.

however I changed the rear brakes and rotors and had a god awful sound. I took apart and noticed the backside of the rotor wearing on this metal on the backside of the rotor, on entire assembly.
Anyone else come across this issue. I did some grinding and got it working but wow how annoying

 

What brand of pad and rotors did you use?

 

I get a hefty discount from Napa, I went with the Napa fleet brakes because this is a work vehicle, and went with the adaptive one coated rotors.
But as I got decent pads and rotors last time I did the swap I was experiencing same issue
